Soon you will leave the Netherlands. Before you leave, you must plan a mandatory pre-check with this form. Are you PhD/Staff/Guest who live at a self-contained studio at Kriekenpitplein, Campusplein, Nieuwegracht and Upsilon? Then please note that this form does not apply to you. You will be informed about your departure in a different procedure, approximately two weeks before your end date.
